华中科技大学硕士学位论文驻店药师药学服务职能现状及对策研究姓名:王梦鸾申请学位级别:硕士专业:社会医学与卫生事业管理指导教师:张新平20080523?_____?“v”12060131231.90.89%85.29%94.71%91.93%91.20%12.71%4.12%0.59%21.08%20.37%56.67%46.67%30%2P0.052.81.67%100%45%83.33%70%5.864.422.24F=199.27P0.05BonfferoniP0.0003.773.152.12F=47.73P0.05BonfferoniP0.000tt10.0026.5771.049P0.0000.0000.30181.80%86.40%80.70%61.60%3.34.1.2.3.4.5.6.4ABSTRACTObjectThepurposeofthisstudyistoexplorethepharmaceuticalcareofpharmacistsinWuhandrugstoresbyobservationandtoanalyzeandsummarizetheinfluentialfactorsofthepharmaceuticalcare.Afterobservationandsystematicalanalysis,suggestionsonbetteringtheservicequalityofpharmacistsindrugstorewillbegiven.DataandMethodsLiteraturereview,observationmethod,DELPHIandstatisticalmethodwereusedinthisstudy.Theobservationmethodwasusedinthenon-participationway,andservicesofthepharmacistsindrugstoresweresurveyedin60pharmacies,whichweredividedintolarge,mediumandsmallsize.Inordertoavoidthedifferencesofpassengerflowwhichwerecausedbygeographicalpositions,thesimilarpharmaciesgeographicalpositionswerechosen.Thepharmaciesincentralareaswereselectedinthisstudy.Theresearchofserviceofeverypharmacistbeganfromtheopeningtimeinthe3days,andlast8hoursineveryday.Thestudywouldbedelayedonedaywhenthepharmacistwasabsent.Thedataincluded:(1)Literaturesconcernthefunctionsofpharmacistsindrugstorebothathomeandabroad.(2)Theregulations,formaldocumentsandmanagementpatternforpharmacistsindrugstore.(3)Dataofpharmacistsindrugstores,includingjobdataandpharmaceuticalcaredataofpharmacistsindrugstoresinWuhan.Thejobdatacontainthefollowingitems:thequalificationsandcertificationownedbythepharmacists,substitutedqualitycontrollerinchargeandthelegalpersonsofthedrugenterprises,thehangingofpharmacists’licensestopractice,thewearingofidentitycardsandtheattendanceofpharmacists’.Thepharmaceuticalcaredatainclude:;thedrugqualitymanagementdataincludethecategorizingofdrugs,medicinestorageandinformationchecking;themedicationguidancedataofmedicalservicesandprescriptionservices;therecommendationofOTCdrugsinantipyreticanalgesicsandanti-inflammatory.ResultsandConclusions1.Thejobdataistheprerequisitefoundationofthepharmaceuticalcare.Thejobdatashowedthat90.89%pharmacistsalsofunctionedduringtheirworkhoursasthequalityresponsiblepersons,therateofHanyang,Qiaokou,JianganandJianghanwas85.29%,94.71%,91.93%and91.20%respectively,while12.71%pharmacistsasthe5businessresponsiblepersons,andtherateofthefoursectionswas4.12%0.59%21.08%20.37%respectively.Thereweresignificantdifferencesamongpharmaciesofdifferentsizeintheaspectofhangingthepharmacists’certificationstopractice,wearingidentitycardsandholdingposts,andthedifferenceswerepositivelycorrelatedwiththesizeofthepharmacies(allP0.05).2.Therateofreflectingdrugs’classificationstoragewas81.67%,wheretheratesofthelargeandmediumsizepharmacieswereboth100%;therateofinformationcheckingwas83.33%,andtherateofpharmaciesofallsizewasabove70%.Themedicineguidancewasstudiedbytheservicelengthperpersonpertimeoftotalmedicationguidanceandthatofprescriptiondrugs’guidance.Thetwoindexeswerecalculatedfromtheservicetimes,servicepersonsoftotalmedicationguidanceandthatofprescriptiondrugs’guidance.Theservicelengthperpersonpertimeoftotalmedicationguidanceofpharmacistsinlarge,mediumandsmallpharmacieswas5.86,4.42and2.24minutesrespectively,andtheservicelengthperpersonpertimeofprescriptiondrugs’guidancewas3.77,3.15and2.12minutesrespectively.Thereweresignificantdifferencesinservicelengthperpersonpertimeoftotalmedicationguidanceandthatofprescriptiondrugs’guidanceamongpharmacistsindifferentsizeddrugstores(Fvaluewere199.27,47.73respectively,allPwere0.000whentheBonfferonimethodwasused),andtheservicetimecorrelatepositivelywiththescaleofdrugstores;Amongpharmacistsindrugstoresoflargeandmediumsize,thereweresignificantdifferencesbetweenthetwoindicators,butnotinthesmalllevel(tvaluewere10.002,6.577and1.049respectively,Pvaluewere0.000,0.000and0.301respectively).Therateofdrugrecommendationwas81.90%,andtherateoflarge,mediumandsmalldrugswas86.40%,80.70%and61.60%respectively.Thereweresignificantdifferencesamongthepharmaciesofdifferentsizeintherecommendationlength,andthedifferenceswerepositivelycorrelatedwiththesizeofthepharmacies(?2=144.888P0.05therankcorrelationcoefficientwas0.162,P0.05).3.Theremainingproblemsincludetheirrationalfunctionofworkingrelatedconditions;thedifficultiesindrugqualitymanagement;thedifferenceofmedicinequalitycontrolexistedamongdrugstoresofvariousscales;introducingmedication;poorservicequalityof6smalldrugs’;limitedcontentsofpharmaceuticalcare;thelackingofcriterionsinstudy;thedifficultiesincombinationofpharmaceuticalcareconceptsathomeandabroad.4.Thefactorsthatplaceinfluencesonpharmaceuticalserviceprovidedbydruggistinpharmaciesincludednationalpoliciesandregulations,departmentsupervision,pharmacist’saccessandeducationsystem,themanagementpatternandmarketingmodesofdrugstores,thewillingnessofpharmacistsandsocialcognition.Suggestions1.Clarifyingtheimportantpositionandservicecontentsofpharmaceuticalcare.2.Reformingtheeducationalsystemaiminga
