Vendor'sQualityCommitment供應商品質承諾書RepresentativesfromSilitekhaveexplainedtherequirementofQualificationandourresponsibilitiestothatprocess.Weunderstandthatthepurposeofthisagreementistoprovideabasisofunderstandingwherebywe,asvendorofmaterialstoSilitek,committodevelopingaplanforprocessimprovementandcontrol,whichwillbeappliedtopartsandorcomponentsproducedforSilitek.Inadditiontocomplyingwiththebasicdefinedelementsofqualification:旭麗的代表已經解釋過品質的需求及我們的責任,我們了解這份合約書是在提供我們可以了解的一個基礎,做為旭麗的一個協力廠商必須發展製程改善與控制的計劃,且適用於零組件的生產,品質要件如下:*WeunderstandandagreetothevendorresponsibilityasdefinedintheSilitekQualityAgreement.我們了解並同意旭麗品質合約書中所載明協力廠商權責。*ForVendorwhohavenotachievedqualificationstatusorwhohavedefectsfoundinprocessorthroughin-cominginspection,weagreetodo100%inspect/testnonconformingparametersuntilprocesscontrolareestablishedandverified.若未能達到品質的要求,及在製程中或進料檢驗發現不良品,我們同意百分之百全檢,一直到製程管制的建立與確認。*WewillpermitSilitekpersonnelaccesstoourfacilitiesforthepurposeofreviewingqualityandmanufacturingprocedures,processesandrecords,exceptthoseareasthatare1)proprietary,2)companyconfidential,or3)governmentclassified.我們允許旭麗人員進入我們的廠內考察我們的品質與製程,但1)公司財產,2).公司機密,3)政府機密不在允許範圍之內。*WewillmaintainproductmeasuringrecordsforallmaterialshippedtoSilitek.Theproductmeasuringrecordsshallinclude,butnotlimitedto,thefollowingitems:1)partnumber,2)POnumber,3)revisionlevel,4)lotsize,5)processcapabilitystudies,6)QualitycontrolCharts,7)quantitiesinspection,8)defectsdescription,9)lotdisposition,etc.我們會保留所有材料測試記錄.這些記錄應包含(但不限制於)以下項目:1.料號2.訂單號碼3.更改序號4.批量5.製程產能研究6.品管圖7.檢驗數量8.不良原因9.批貨處置*WeunderstandthatSilitek'sQualityPlanistoachievethetotalcustomersatisfactionthematerialVendorproducedforSilitekshallimplementtheprocessCpkstudyonkeycharacteristicoftheprocess.我們了解旭麗品質計劃是要達到全面客戶滿意標準,協力廠商必需應貫徹實行主製程CPK之研究。*WeunderstandthatSilitek'sfunctionalrejectrateformaterialsusedinproductassemblyshallcomplyto=________Dppmintotalprocess.我們了解在所有製造過程中,使用的材料電性不良率應=________Dppm*WeunderstandthatMIL-STD105EAQLof________%orequivalentstandardistobeusedatourfacilitypriortoshippingtoSilitek.我們了解產品出貨給旭麗之前,我們廠內應使用MIL-STD105EAQL_______%或相當之標準。*WeunderstandthatqualifiedmaterialwillbeshippedtoSilitekand,shouldanydefectbefound,weassumeliabilityforscreening,rework,credit,orreplacementasdescribedinQualityAgreement.我們了解交給旭麗之貨品必須是合乎標準的,如有任何不良品,我們會承擔責任去做檢驗、重工、扣款或換貨,如品質合約書所訂。*Silitek'sacceptancelevelfordeliveryperformanceshallbe100%ontime,asdefinedbyindividualplant'sneed.依據各廠需求所訂,旭麗要求交期是100%準時。Company(公司):Signatureby(簽章):Date(日期):Title(職稱):StandardTitle標準書名稱Code編號Version版本Page頁數ReleaseDate12/22/20001.01/107/15/1997SilitekCorporation旭麗公司Vendor'sQualityCommitment供應商品質承諾書ModifyDateStdDocumentType標準書分類Procedure規定
