BN-ZY-XB-15/A-0-1-编号：__________________百年职校义务工作者服务声明BBNNVVOOCCAATTIIOONNAALLSSCCHHOOOOLLVVOOLLUUNNTTEEEERRAAGGRREEEEMMEENNTTFFOORRMM甲方：_____________________住所地：_________________Location:______________________________________________________乙方：姓名Volunteer’sName：联系电话Telephonenumber：甲方是专门为贫困家庭子女提供免费职业教育的公益学校，学校资金主要来自企业和社会组织资助。为帮助贫困农民工和贫困家庭子女接受职业教育和获得就业机会，为建立和谐社会贡献力量，乙方自愿无偿担任甲方义务服务工作。甲方感谢乙方的志愿服务。经友好协商，双方就该义务工作共同声明如下：BNVSprovidesfreevocationaleducationtochildrenoffamilieslivingbeneaththepovertyline.Fundingfortheschoolisentirelyprivate,stemmingfromgrantsanddonationsmadebycorporations,foundations,foreignsponsors,andindividuals.BNVSwelcomesqualifiedvolunteerstohelpeducatethestudents,whosegoalistobegainfullyemployedupongraduation.BysigningthisAgreement,theapplicantagreestoabidebythetermsandconditionsassetforthinthisAgreementandintheBNVSVolunteerManual.1、乙方义务工作岗位为Volunteerposition：_________________。2、乙方义工工作期间为____年____月____日至____年____月____日（若长期为百年职校工作，请空白不填）。具体工作时间，根据甲方安排另行确定。Thevolunteeragreetoworkfrom(year/month/day)__________to(year/month/day)__________.DetailedworkingtimeisarrangedbyBNVS.3、甲方负责为乙方提供必要的工作环境和条件，并根据乙方需要如实提供义工工作证明。BNVSagreestoprovideanappropriateworkingenvironmentforthevolunteer,aswellprovidethevolunteerwithaletterdocumentingtheirworkuponrequest.4、由于甲方从事的是慈善事业，乙方从事的是义工服务，甲乙双方不存在劳动合同关系，甲方不负责乙方的保险和福利。乙方在从事义工工作中发生的任何意外造成人身和财产损坏由乙方自行承担。BNVSwillnotbeheldaccountableforanydamage,injury,orlossexperiencedbythevolunteerasaresultoftheirworkatBNVS.5、甲方保证安排乙方从事的义工工作符合各项法律法规，乙方已经阅读和了解了甲方各项规章制度，同意遵守甲方的各项规章制度，确保工作质量符合甲方的要求。BNVSguaranteesthattheworkundertakenbyvolunteersadherestothelawsandregulationsofthePRC.ThevolunteeragreestoadheretotherulesandregulationsofBNVSandthattheirworkwillmeetsthestandardsofBNVS.BN-ZY-XB-15/A-0-2-6、凡从事与学生接触的义工工作，为保证学校教育工作的一致性、学生平等权利和各方的安全，乙方应在学校内与学生见面和交流,与学生的接触交流应及时向校方通报。如带学生课余外出参加活动须事先向学校教导处申请，教导处同意且得到家长或监护人同意后方可带出学校。乙方不得带学生到个人居所，不支持或安排学生在外留宿，不到高档餐厅就餐，不到网吧、酒吧、歌厅等娱乐场所，不赠送奢侈礼品，不要将个人住址、家庭电话等私人联络方式告诉学生，不得到学生家庭留宿。Thevolunteerguaranteestoofferequalopportunitiestoallstudents.Allexchangesormeetingsbetweenvolunteersandstudentsshouldtakeplaceattheschool.Anyactivityoutsideofschoolrequirespriorpermission,viatheBNVSstudentoffice,fromthestudents’parents’orguardians’permission.Volunteersareprohibitedfrominvitingstudentstotheirpersonalresidencesorformakingarrangementsforstudentstosleepelsewhere.Volunteersshouldnottakestudentstoexpensiverestaurants,bars,internetcafes,orconcertswhichportrayanupscalelifestyle.Volunteersmustalsonotgivepersonalcontactinformationtostudents(address,phonenumber,oremailaddress).Volunteersarealsonotpermittedtospendthenightinstudents'homes.7、乙方是否同意甲方在学校的各类出版物/网站上公布乙方姓名和个人照片？（）同意（）不同意Doesthevolunteer()allowBNVStousetheirnameandphotographinschoolrelatedpublications.()NOTallowBNVStousethevolunteersnameandphotographinschoolrelatedpublications.8、甲方建议乙方将在在百年职校担任义工工作的事情告知本单位，以免引起不必要的纠纷。Thevolunteerwillinformhis/heremployeraboutvolunteeringatBNVS,inordertoavoidunnecessarydisputes.9、所有涉及乙方义工工作的图片、音像资料以及乙方工作期间在学校创作完成的作品，甲方有权在教学和学校宣传工作中无偿使用，不构成对乙方肖像权、知识产权及其他任何人身和财产权利的侵害。乙方在非甲方义工工作范围使用学校任何教学和学生资料均需得到甲方的书面同意。BNVSretainstherighttoallintellectualpropertycreatedbyvolunteerswhileatBNVS(includingphotos,audio,video,text,designetc.).AllinformationregardingtheschoolandthestudentsusedbythevolunteerissubjecttothewrittenpermissionofBNVS.10、乙方违反上述声明，或做出其他有损甲方声誉行为的，甲方有权利单方决定中止乙方在甲方义工工作，并不承担任何责任。AnyviolationoftheaboveregulationsoranyotheractiononthepartofthevolunteerthatadverselyaffectsthereputationofBNVSwillbecausefortermination.我已阅读上述声明并同意遵守。IagreetotherulesandregulationsofBNVocationalSchoolasstatedabove.乙方签名VolunteerSignature：日期（年-月-日）Date(year/month/day)：
